Enclosed space entry accidents are the leading cause of fatality on cargo ships. Cargo holds, ballast tanks, cofferdams, pump rooms, void spaces, chain lockers, and inert-gas-blanketed tanks routinely contain atmospheres that will kill a rescuer within seconds of entry — usually oxygen deficiency, sometimes toxic gas accumulation from cargo residues or rust reactions. The high fatality rate reflects two persistent patterns: (a) crew members entering without proper atmosphere testing, and (b) multiple deaths from untrained rescuers attempting to save the initial casualty. The SOLAS XI-1/7 regulation and IMO Res. A.1050(27) codify the procedural response.
The atmosphere hazards
Oxygen deficiency. Normal atmospheric O2 = 20.9%. Impairment starts below 19.5%; unconsciousness below 12%; rapid death below 8%. Rust reactions in ballast tanks + cargo respiration in bulk holds (grain, coal, wood chips) can drop O2 to 4-8% inside days.
Toxic gases. H2S from crude cargo, phosphine from fumigated grain, CO from smouldering coal, HC vapours from tanker cargo, benzene from crude, mercaptans from chemical cargo. Some are lethal at ppm concentrations (H2S death threshold ~500 ppm; phosphine death ~200 ppm).
Inert-gas atmospheres. Product/chemical tanker cargo tanks are typically inerted with N2 or exhaust gas at O2 < 5%. Instantly fatal on entry without SCBA.
Flammable atmospheres. Hydrocarbon or chemical vapours can be at explosive concentration; entry with electrical or spark-producing equipment is prohibited.
SOLAS XI-1/7 — Atmosphere testing instrument
Since 1 July 2016 (SOLAS XI-1/7 as amended), every ship subject to SOLAS Ch. XI must carry a portable multi-gas atmosphere-testing instrument capable of measuring at least oxygen, flammable gases/vapours, hydrogen sulphide, and carbon monoxide. The instrument must be maintained + calibrated per manufacturer's instructions with the calibration log available for PSC inspection. Two instruments are commonly carried for redundancy.
Enclosed Space Entry Permit
Before entering any enclosed space, an Enclosed Space Entry Permit must be raised, ventilated, tested, signed, and posted at the entry point. Required content:
Space identification. Which space is being entered (e.g. "No. 3 starboard ballast tank").
Purpose of entry. Specific task (inspection, cleaning, maintenance, repair).
Ventilation record. Duration of forced ventilation + fan flow rate. Minimum 4 air changes before initial test.
Atmosphere test results. O2, LEL/UEL flammability, H2S, CO tested at multiple depths (top, mid, bottom of space) + timestamp.
Personnel entering. Names of entrants + designated attendant (buddy).
Communication + rescue equipment. Radio channel, safety harness + line, portable-tester on each entrant, rescue plan.
Duration of validity. Permit typically valid for a single shift; re-testing required if the atmosphere may change (e.g. cargo work in adjacent hold, changes in ventilation).
Force-ventilate the space. Minimum 4 air changes; typically 30-60 minutes depending on space volume + fan capacity.
Test from the entrance. Do NOT enter to test. Extend the sample probe on a hose into the space.
Test at multiple levels. Top, mid, and bottom of the space. Heavier-than-air gases (H2S, HC) collect at the bottom; O2 depletion can occur anywhere.
Verify normal atmosphere. O2 = 19.5-23.5%, LEL < 10%, H2S < 5 ppm, CO < 25 ppm are typical thresholds. Any anomaly = further ventilation + retest.
Continuous monitoring during entry. Each entrant carries a personal portable multi-gas tester with audible alarms.
Re-test if the space is exited and re-entered. Even a brief exit voids the permit.
Rescue procedure — the multi-fatality prevention
The single most important lesson from the fatality statistics: DO NOT ENTER TO RESCUE.Multi-fatality enclosed space accidents follow a stereotyped pattern: initial victim collapses (typically from O2 deficiency), colleague enters to rescue and immediately succumbs, second rescuer enters and dies, third rescuer... Enclosed space fatalities routinely claim 3-5 crew from one incident because untrained rescuers cannot resist entering to save colleagues visibly incapacitated below.
Correct rescue procedure:
Attendant does NOT enter under any circumstances. Attendant calls the alarm.
Rescue team dons breathing apparatus (SCBA) with full face mask. This takes minutes — meanwhile the initial victim may not be recoverable.
Rescue team enters with SCBA + retrieval harness. Attach retrieval line to victim.
Extract victim via the retrieval line without additional entry personnel.
Administer basic life support to victim on deck; call medical evacuation.
Regular drills of this procedure are mandatory under SOLAS III/19. Master should ensure every crew member understands the "do not enter" rule to bone-deep instinct.
Cargo-specific risks
Grain + wood chip + coal. Cargo respiration + microbial activity consumes O2. Recently loaded holds should be treated as unsafe. Coal also produces CH4 + CO.
Fumigated grain. Aluminium phosphide fumigation releases phosphine gas over 5-14 days. Fumigated holds must not be entered without extended off-gassing + verification.
Product tanker cargo holds. HC vapour retention post-discharge; inerted atmospheres.
Ballast tanks. Rust consumes O2 continuously. Long-empty ballast can have O2 < 10% + significant CO2 accumulation.
Chain lockers + rope stores. Rust + limited ventilation; O2 deficiency is common cause of fatality here.
