Why dental problems at sea become a sign-off issue — and what to fix ashore before flying.

A throbbing molar in the middle of an Indian Ocean transit cannot be fixed by the second officer's first-aid kit. Mid-contract dental emergencies are one of the most common causes of unscheduled deviation and unplanned sign-off. P&I cover for shore-side dental treatment is often limited, and a deviation to a port for dental treatment can be charged back to the seafarer in some cases. A single ashore appointment before joining is the most cost-effective health insurance a working seafarer can buy.

Practical steps

• Book a dental check at least 6 weeks before flying — time to schedule any follow-up work.
• Treat anything borderline: an old filling that is starting to crack will fail at sea.
• Ask the dentist for a written summary of any ongoing watch points.
• Pack ibuprofen, paracetamol, clove oil, and temporary-filling material.

When to escalate

• If pain becomes constant onboard — report to the Master, request TMAS consultation, and ask the agent to arrange a dental appointment at the next port.
• If swelling, fever, or difficulty swallowing — treat as a medical emergency.
