Why it matters

Every reported near-miss saves the same incident from happening with worse consequences next time. ISM Code makes the system mandatory — but the culture has to make it safe.

Common hazards

• Under-reporting from fear of blame.
• Reports filed but not acted on.

Practical checklist

• Report all near-misses, however minor.
• Describe what happened, what could have happened, what controls failed.
• Suggest a control that would prevent it next time.
• Read what the office did with the last batch — close the loop.
