Why honest reporting protects you and your shipmates.
Why it matters
Every reported near-miss saves the same incident from happening with worse consequences next time. ISM Code makes the system mandatory — but the culture has to make it safe.
Common hazards
- Under-reporting from fear of blame.
- Reports filed but not acted on.
Practical checklist
- Report all near-misses, however minor.
- Describe what happened, what could have happened, what controls failed.
- Suggest a control that would prevent it next time.
- Read what the office did with the last batch — close the loop.
