Statement of Facts (SoF) template

The Statement of Facts is the chronological record of all material events at a port call — pilot on/off, NOR tendered, all-fast, cargo operations start/finish, weather stoppages, surveys, sailing. It is the primary evidentiary document for laytime and demurrage calculation under the charter party.

Conventions to follow

All times in port's local time, with UTC noted in the remarks column when there is risk of confusion (Suez Canal, Panama Canal transits).
Weather stoppages: log start time, end time, and a brief reason. Master's opinion controls — but cite the wind-force / sea-state from the bridge log.
Sign-off — Master countersigns; Agent countersigns. Owners' copy + Agent's copy + Charterer's copy at minimum.
